Customer Rewards Terms and Conditions

The following Terms and Conditions apply to the Crystal Rewards Program. The Crystal Rewards Program is offered only to dental offices which purchase Crystal Tips only through our on-line store. As used herein, the term "customers" refers to eligible dentists or their dental offices who have an active account either with Crystal Tip, or an authorized representative or distributor. Eligibility for this program is limited to one recipient per billing account. Dental offices with more than one dentist are eligible for points based on their accounts. If there is one account for the location, that is the only account to receive Crystal Rewards points. If each doctor has a separate account, even though the shipping address is the same, each doctor will be eligible for the Crystal Rewards Program under that doctor's account. Crystal Rewards Program points are available on qualifying purchases of Crystal Tips and all other products in the Crystal Tip store. Only purchases made directly through the on-line store will accrue Crystal Rewards points. There is no limit to the number of points customers may accrue. However, if at anytime Crystal Tip Rewards program is terminated, no additional points may be accrued from the date of program termination, and customers will have up to 90 days to redeem their current accrued points. Points will not be issued for freight or tax charges in connection with purchases. Point totals will be maintained on customer accounts and may be reviewed by customers on-line at www.crystaltip.com by clicking on the Rewards icon on the Crystal Tip home page, after customer has logged into the proper account with username and password. Westside Resources reserves the right, at its sole discretion, to terminate the Crystal Rewards Program, or add program rules, regulations, terms and conditions, including, but not limited to, imposing time limits and changes in point values, rewards and reward levels without prior notice. Should the Crystal Tip Rewards program be terminated, all customers will be notified that they have up to 90 days in which to redeem their accrued points. Crystal Rewards Program points have no cash value and may be redeemed only for products offered in the Crystal Rewards catalog, or for Crystal Tips. Crystal Rewards points may not be redeemed for account credit. Points earned by the qualifying purchase of products from/by Westside Resources, as described herein, will be credited to the "billing" or "bill to" customer account. Any evidence of fraud or attempted fraud by or on behalf of a customer account shall result in disqualification of that account, and shall void all points earned by that account. Crystal Rewards are not transferable; no substitutions allowed at the request of customers or their accounts. Westside Resources reserves the right to substitute rewards of equal or greater value if an advertised product (reward) becomes unavailable. All point redemptions are subject to verification by Westside Resources. Allow approximately 6-8 weeks for shipment of rewards following verification. Delinquent accounts will not be eligible to redeem Crystal Rewards points until account has been brought current. SHIPPING INFORMATION: Unless specifically indicated otherwise on the Crystal Rewards item, shipping is NOT included in your Crystal Rewards redemption. Your credit card will be billed for exact shipping costs only. We will notify you in advance and let you know the exact cost for shipping.
